Catholic Charities bridges the gaps in elder care by providing tailored in-home support, specialized hospital-to-home transitional care, essential transportation, and dedicated advocacy to help local seniors safely maintain their independence.

At MSSP, we believe in the dignity of aging in place. Our dedicated team of Registered Nurses and Social Workers collaborates to provide a comprehensive interdisciplinary approach to care. By identifying and coordinating key community resources, we help frail seniors remain safely in their own homes and delay the need for nursing home placement.

Homemaker Program

Helping seniors maintain their independence and stay safely in their own homes. This program provides essential, non-medical support to older adults across Stanislaus County by offering light house cleaning, laundry assistance, grocery errands, and meaningful companionship.

Supporting independence and dignity for older adults (ages 60+) across San Joaquin County. The Title III Program provides essential, non-medical in-home such as light housekeeping, personal care, or respite care to help seniors age safely in the comfort of their own homes.

Providing reliable, assisted transportation throughout the county for individuals aged 60 and older, as well as adults living with disabilities. We help you stay mobile and connected, with highest priority always given to scheduling your essential medical appointments.

Recovering after a hospital stay can be overwhelming. Our Transitional Care Services bridge the gap between discharge and a full, strong recovery. Our specialized team handles the logistics so you or your loved one can focus entirely on healing. We coordinate follow-up medical appointments, secure reliable transportation, and seamlessly connect you to the essential health and well-being resources you need right at home.

Stanislaus Elder Abuse Awareness Program
SEAPA

Protecting and empowering older adults and dependent adults in our community. We provide dedicated outreach and advocacy, while collaborating with local partners to host educational workshops. Our goal is to equip seniors with the tools to prevent fraud and exploitation, while offering compassionate guidance and resources to help those who have been victimized.

Long-Term Care Ombudsman Program

Our trained, state-certified staff and volunteers serve as dedicated advocates for residents living in skilled nursing homes, residential care homes, and long-term care facilities. We protect seniors' rights, resolve complaints, and champion the highest quality of life and care for those who call these facilities home.

Elder Abuse Prevention Program

Protecting seniors starts with education. We offer comprehensive Elder Abuse Prevention and Mandated Reporting training for businesses, healthcare facilities, and agencies whose staff or volunteers interact with older adults in any capacity. This vital training teaches your team how to identify the signs of elder abuse and properly report suspected cases. Multipurpose Senior Services Program (M.S.S.P.)

Keeping Seniors Safe and Independent at Home.

MSSP utilizes an interdisciplinary team of RNs and Social Worker Case Managers to connect frail older adults with the community resources they need to thrive at home, successfully delaying or preventing nursing home placement.

Hospital-to-home service that bridges the gap between a hospital discharge and a strong recovery. Transitional Care Specialists help patients secure arrangements to medical appointments and links patient to resources related to health and well being.

(SEAPA)

Provides outreach and advocacy on behalf of seniors and dependent adults.  Collaborates with community partners to provide educational workshops aimed at helping seniors to avoid becoming a victim and helping victims
