Program: Multipurpose Senior Services Program (MSSP) Position: Nurse Care Manager (NCM) Salary: 46.35/hr. to $47.28/hr. Part-Time Seeking a Nurse Care Manager (NCM) to join our Multipurpose Senior Services Program (MSSP). This is a Part-time position (25 hours per week). The NCM is responsible for conducting in-depth assessments/reassessments of participant's medical, physical and health needs to ensure eligibility for Medi-Cal. The NCM works closely with Social Worker Care Managers (SWCM) and certifies the level of care of assigned participants. Job responsibilities include: physical and cognitive assessments as necessary and interpreting clinical findings. Works closely with the Supervising Care Manager, Social Work Care Managers, Physicians, and other community healthcare professionals. Certifies level of care, together with SWCM’s develops individualized care plans, monitors participants needs and provisions in the care plan, updates participant case file on a timely bases. NCM must be willing to travel throughout the San Joaquin County to work with participants in the community. Desired qualifications: Must possess a current CA RN License and in good standing. Experienced working with seniors and case management. Must be able to communicate effectively both verbally and in writing using online database.
Program: Multipurpose Senior Services Program (MSSP) Position: Social Worker Care Manager (SWCM) Job Type: Full-Time 40 Hours per week Salary: $22.66 per hour Catholic Charities Diocese of Stockton’s Multipurpose Senior Service Program (MSSP) has an opening for a Full-time Social Worker Care Manager (SWCM) position, 40 hour week. The MSSP SWCM provides a wide variety of care management services to a diversified participant population of older adults living in San Joaquin County utilizing a comprehensive psychosocial services model. MSSP provides in-home care management and coordination to participants who are 60 and older and certified eligible for a skilled nursing home. Major Responsibilities/Duties: • Conducts in-depth assessments/reassessments covering psychosocial, rehabilitation, and environmental concerns • Provides home safety checks and quarterly home visits for a monthly case load of 50–53 clients • Collaborates and consults with Register Nurse and Supervising Care Supervisor in the development of the participants individualized care plan • Arranges, coordinates, and verifies services/interventions identified on the care plan • Refers participants to specialized consultants to provide detail evaluations and recommendations to improve the participants functional level • Identifies and develops support systems for the participant • Follows strict case recording and reporting established by CDA • Ensures that prior to purchasing and authorizing waiver services all other resources have be exhausted • Participates in interdisciplinary monthly meetings and conducts peer reviews as assigned • Interacts with various community agencies and service vendors The MSSP SWCM duties described above are supported by, but not restricted to other related professional duties. Desired Qualifications: • Bachelor’s degree in psychology, counseling, rehabilitation, gerontology or sociology and two years experience working with older adults. • Must be able to pass live-scan (FBI and DOJ) • Able to adhere to strict confidentiality in accordance with HIPAA regulations • Computer literate and have the ability to learn new computer applications • Excellent communication, organizational skills and time management • Must be able to sit, stand, walk, push/pull, bend, reach, overhead and lift up to 25 pounds • Current CA driver’s license, have reliable transportation and insured vehicle • Ability to travel within San Joaquin County • Fluency in English and Spanish is highly desirable
