Program: Supportive Services for Veteran Families Position: Program Manager Job Type: Full-Time 40 Hours per week Salary: $36.56-37.96 Hr. Catholic Charities Diocese of Stockton is seeking a full-time Supportive Services for Veteran Families (SSVF) Program Manager to oversee the implementation and delivery of the SSVF program serving homeless and at-risk veterans throughout San Joaquin, Stanislaus, Calaveras, Amador, Tuolumne, and Mariposa counties. The Program Manager will provide leadership and oversight of program staff and services; ensuring program goals and requirements are met. The position will work collaboratively with the Department of Veterans Affairs (VA) and community partners to support veterans and their families. The Program Manager will also be responsible for program administration, staff supervision, service coordination, and maintaining effective relationships with community and veteran-serving organizations. Duties and Responsibilities •Incumbent will provide oversight of program goals and objectives, ensuring compliance with grant requirements, which includes supervision of the case managers and other program staff. •The Program Manager will work closely with the Executive Director to support the preparation and submission of annual grant renewals and applications for the SSVF program, including gathering program data, outcomes, supporting documentation, and other information required for grant submissions. •The Program Manager ensures adherence to screening and intake processes by which eligible Veterans and their families will be assessed for services, as well as appropriate documentation for clients’ files. •Incumbent will oversee the development and/or distribution of materials such as brochures, flyers, posters etc. to be utilized for outreach and education regarding the program. •The Program Manager will build networking relationships in the community with other providers to enhance the services for veterans and their families. •Incumbent shall submit required reports and evaluations to the VA as the grant may require, gathering the necessary data to complete outcome reports. •Incumbent is responsible for reviewing monthly budget reports, assisting the Finance team with appropriate fiscal management. •The successful candidate must be willing and able to travel as needed to meet program, needs. Desired Qualifications 1.Master’s degree in social work, preferred, or a Bachelor of Science with 3-5 years’ experience in the social services sector. 2.Experience with Veterans Affairs highly recommended. Administrative skills including budgeting principles. 3.Able to work collaboratively with partner organizations and represent SSVF at public meetings. 4.Supervisory experience -three years desirable. 5.Ability to communicate effectively, both verbally and in writing, with staff, and community partners. 6.Proficient computer skills, including the ability to navigate electronic databases, case management software, and Microsoft Office applications. 7.Ability to maintain confidentiality and exercise sound judgment when working with sensitive information. Program: Supportive Services for Veteran Families Position: Case Manager Job Type: Full-Time 40 Hours per week Salary: $24.00 per Hr. Catholic Charities is seeking a Case Manager in the Supportive Services for Veterans and Families (SSVF). This is a full-time position based out of our Stockton office. The Case Manager will assist low-income homeless veterans and veteran families at risk of becoming homeless by providing case management and housing stabilization services. A candidate must have a commitment to and compassion for the needs of low-income populations. The responsibilities include Conducts in-depth assessments/reassessments covering veteran’s needs and rehousing stability. Housing advocacy & intervention to assist veteran families maintain their current housing or locate affordable housing. Refers clients to other agencies for services not available within Catholic Charities. Desired qualifications: Preferred qualifications include knowledge of Homeless Management Information System (HMIS) data entry, Continuum of Care (CoC), Coordinated Entry System (CES). Strong verbal and written communication skills, ability to do community outreach and to community organizations and case management. Must have a clean DMV record with the ability to travel throughout San Joaquin, Stanislaus, Tuolumne, Mariposa and Calaveras counties, our service areas. Program: San Joaquin Transportation Position: Driver Salary: $18.48/Hr. Catholic Charities is seeking a Driver in the San Joaquin Transportation Program. This is a Part-time (20 hrs.) position. This position assures elderly at risk clients have transportation to the grocery store and medical appointments. Responsibilities: Include transporting senior clients to designated destinations. Treats clients with courtesy, care and compassion. Provides safe, secure, and dependable travel to clients’ destinations. Driver makes sure clients are always buckled into their seats and wheelchairs are safely secured. Participates in mandated reporter training on elder abuse issues and reports to appropriate agencies. Notifies coordinator if client does not answer door, ill, lost or hurt. Inspects vehicle daily and reports any maintenance problems or concerns to the coordinator. Desired Qualifications: Excellent driving record. Must have a valid California State Drivers’ License and auto insurance. Demonstrated commitment working with seniors (patience, understanding, caring)—ability to speak English and Spanish.
Program: Title III Position: Caregiver (San Joaquin) Salary: 17.38/hr. Catholic Charities is seeking a Caregiver in the Title 3 B&E programs. This is a Part-time position. This position is responsible for providing direct care to seniors living in their own homes who are 60 years of age and above who are enrolled in the Title 3 programs and reside in San Joaquin County. The program components include: light housekeeping, respite care for the frail elderly, Caregiver Respite for an unpaid primary caregiver, and personal care. Responsibilities include: Provide non-medical stand-by assistance with personal care if/as assigned, Identify and document client special needs/problems observed or expressed by client and report to Title IIIB Coordinator, Prepares meals if requested by client. Desired Qualifications: Able to perform light housekeeping duties, possession of driver’s license and ability to see clients in San Joaquin County as assigned by the Coordinator, and able to pass background checks through the DOJ and FBI clearance.
