Incumbent must have a commitment to and compassion to meet the needs of low income and culturally diverse populations. The NCM is responsible for evaluating potential clients as a total person and identify the functional and health limitations that impede independent living.


Responsibilities include:

  • Conducts in-depth assessments/reassessments on clients covering medical, health and rehabilitation concerns.
  • Certifies level of care.
  • Performs physical assessments as necessary and interpreting clinical findings.
  • Works closely with the MSSP Clinical Supervisor, Social Care Managers, Physicians, and other healthcare professionals
  • Participates in developing care plans and implementing the services: case recordings and reporting. 
  • Conducts follow-up and monitors the client’s needs and care plans.
  • Develops support systems for the client

  • Must possess a current CA RN License
  • Bachelor’s Degree in Nursing -preferred
  • Experienced working with seniors and with care management.

This position is funded by a federal grant requiring the operation of a Naturalization program in Stockton and Modesto. Coordinator will work at both sites weekly totaling 40 hours. Stockton school is M &W 5:30 – 9:00 pm, Modesto School is T & TH 5:30-9:00 pm and all other hours worked at Stockton Office. Coordinator is responsible for the daily operation of Naturalization program. Duties include but not limited to; providing intake and revision of enrollment for each student, ensure each student file is updated with appropriate signed documents, test scores and level of class, preparing sign-in for attendance of Instructors/Teachers and students, will work closely with Program Manager in providing statistics/data of attendance, hours of class, CASAS testing and benchmark data of each student for reporting purposes.
