CALFRESH OUTREACH PROGRAM -APPLICATION ASSISTOR (STANISLAUS COUNTY) 

Catholic Charities Nutrition Assistance Services has a part-time opening for a CalFresh Outreach-Application Assistor in the Modesto office. This position provides outreach activities to increase awareness of CalFresh benefits in Stanislaus County, in both English and Spanish. The position will require the incumbent to work 29 hours per week, and may include occasional evenings and weekends. Applicants must be able to work independently and occasional evenings or weekends, have excellent communication skills, close attention to detail, willing to speak in front of large audiences, and possess valid CA driver’s license and willingness to drive own vehicle to events. SENIOR SUPPORTIVE SERVICES - TRANSITIONAL CARE SPECIALIST 

Catholic Charities of the Diocese of Stockton is seeking to fill a Full-Time Transitional Care Specialist (TCS) position in Stockton, CA. The TCS will provide adults, seniors, and their caregivers with referrals to social service organizations to avoid re-hospitalization after a discharge from the hospital. The TCS will coordinate, advocate and provide services that will meet each client’s needs. This may require coordinating and assisting clients with scheduling transportation, medical appointments, medication delivery, in-home care, establishing follow-up care and/or obtaining other community resources.  The TCS will keep track of clients via database, monitor re-admissions, etc.  The TCS must have strong verbal and written communication skills in English, and knowledge of community resources. This position requires a valid California Driver’s License and reliable transportation. Qualifications include: A Bachelor’s degree in Sociology, Psychology or social work or a minimum of two years’ experience in case management with adults and/or seniors. SENIOR SUPPORTIVE SERVICES -TRANSPORTATION PROGRAM COORDINATOR

Catholic Charities of the Diocese of Stockton has an immediate opening in the Stockton office for a Full-time Transportation Program Coordinator.  The Coordinator is responsible for providing non-emergency medical and grocery shopping transportation for vulnerable elders in San Joaquin County. This position includes administrative duties, data entry tasks, customer service, and working with volunteers.  This position requires conducting initial intake and screening of applicants, completing the intake and application process, collecting data, completing monthly reports and assisting with driving clients when needed.   This position requires the ability to work independently, exercise sound judgment, time management, and initiative.  In addition it requires treating older adults with patience, understanding and compassion.  Proficiency in Microsoft Windows, Microsoft Excel and general internet use required.  Must have reliable transportation, valid California Driver’s License, excellent driving record, and be bilingual English-Spanish .  This is a non-exempt, 40 hour per week position, with full benefits.

Catholic Charities of the Diocese of Stockton has an immediate opening in the Stockton office for a Part-time (20 hours) Transportation Program Driver.  This position requires driving and assisting vulnerable elders to non-emergency medical appointments and grocery shopping in San Joaquin County.  Assistance with walkers, wheel-chairs, and groceries is also required.  In addition, it requires treating older adults with patience, understanding and compassion.  This position requires the ability to work independently, exercise sound judgment, time management, and excellent customer service.  Must have reliable transportation and valid California Driver’s License.  Bilingual, English-Spanish preferred, but not required.

IMMIGRATION LEGAL SERVICES -CASE MANAGER

Catholic Charities of the Diocese of Stockton is seeking a full-time Case Manager to provide assistance in immigration legal services, and administrative support for the Immigration Program. Case management duties include, but not limited to; interviewing clients regarding their legal immigration status, assisting clients with immigration forms and processes while also providing administrative duties. Candidate must be bilingual (English/Spanish) with strong interpersonal, written and oral communication skills as well as proficient in computer skills. Candidate will have a positive and professional attitude, flexible, attention to detail and sensitive to the needs of culturally and economically diverse client populations. While no immigration work experience is required, candidate will preferably completed courses/hold degree in Law, Criminal Justice, Sociology, Social Work or related field. Salary will be competitive and dependent on experience. SENIOR SUPPORTIVE SERVICES -MSSP SUPERVISING CARE MANAGER

Catholic Charities of the Diocese of Stockton is seeking to fill a full-time MSSP Supervising Care Manager position.  Under the direction of the Site Director, the SCM is responsible for planning, organizing, developing and implementing the principles and techniques employed in a comprehensive care management model for the frail elderly.  The SCM position combines both administrative duties and clinical supervision responsibilities.  Requirements include: A Master’s degree in social work, nursing, psychology, counseling, rehabilitation, gerontology, or sociology.  Minimum of one year Supervisory experience. Minimum of two years’ experience working with the aging population (65 years and above). Minimum of one year care management experience. Knowledge of services available to seniors in the community. Strong interpersonal, organizational and communication skills. Must be able to occasionally work non-traditional hours, overtime, and travel as needed for trainings and conferences.
