MULTIPURPOSE SENIOR SERVICES PROGRAM - NURSE CARE MANAGER

Catholic Charities of the Diocese of Stockton has an immediate opening for a part-time Registered Nurse (RN) in the Multi-Purpose Senior Services Program. The RN is responsible for evaluating and determining client eligibility, conducts assessments involving health and mental concerns of seniors, determines and certifies level of care plans, works closely with social worker case managers, Clinical Supervisor, physicians and other health care professionals, monitors client needs and care-plans, conducts home visits, records case records and conducts case audits. Must possess current CA RN License, Bachelor’s Degree in Nursing, experience working with seniors, have valid CA Driver’s License, CPR and First Aid certified, able to pass Live Scan (FBI and DOJ) and be computer literate (Word and Excel). THE HOMECOMING PROGRAM -TRANSITIONAL CARE SPECIALIST

Catholic Charities of the Diocese of Stockton is seeking to fill a Full-Time Transitional Care Specialist (TCS) position.  The TCS will provide adults, seniors, and their caregivers with referrals to social services; and coordinate transportation, medication delivery and in-home care services to avoid re-hospitalization after a discharge from the hospital.  The TCS must have strong verbal and written communication skills in Spanish and English, and knowledge of resources in the Stockton communities.  This position requires a valid California Driver’s License and reliable transportation.  Qualifications include: A Bachelor’s degree in Sociology, Psychology or social work or a minimum of two years’ experience in case management with adults and/or seniors. FAMILY WELLNESS -PREVENTION AND EARLY INTERVENTION (PEI) PROGRAM -PROGRAM ASSISTANT

Catholic Charities of the Diocese of Stockton is seeking to fill a full-time Counseling PEI Program Assistant position in Stanislaus County. The Program Assistant will conduct client intake, and assist in the coordination of clinical and licensed staff within the program.  The program assistant provides administrative support to the Program Manager for  connecting with parishes, community and government agencies to provide effective prevention and early intervention counseling services . The program assistant must have strong verbal and written communication skills in Spanish and English, and knowledge in mental health community resources. This position requires a valid California Driver’s License and reliable transportation. PARENT SUPPORT PROGRAM -CHILDCARE

Catholic Charities of the Diocese of Stockton Parent Support program is seeking a motivated individual to provide Childcare services at various locations throughout San Joaquin County. The Parent support program offers childcare services to parents who have children under the age of 11. This position is part- time, maximum of 8 hours per week. 

Qualifications/Requirements:

  • Experience in working with children (5 to 11 years of age).
  • Be dependable, have good communication skills, and have excellent work ethic.
  • Ability to accept guidance and supervision.
  • Over 18 years of age.
  • Dependable transportation.
  • Must complete criminal background check and child sexual abuse prevention trainings before beginning work.

ESL/CIVICS TEACHER POSITION

Catholic Charities ESL/CIVICS program is seeking to hire a part time ESL/Civics teacher to provide class on Tuesday and Thursday evenings from 6:00- 9:00 PM at Mark Twain Jr. High School, 707 Emerald Ave, Modesto CA.

The Incumbent is to teach and prepare Lawful Permanent Legal Residents for the Civics (US History and Government) and English (Reading, Writing and Speaking) components of the Naturalization Exam.  Must have a valid California Teacher's Certificate in English as a Second Language and/or experience with teaching ELL.  Catholic Charities will provide curriculum.
