The Coordinator is responsible for providing non-emergency medical and grocery shopping transportation for vulnerable elders in San Joaquin County. This position includes administrative duties, data entry tasks, customer service, and working with volunteers. This position requires conducting initial intake and screening of applicants, completing the intake and application process, collecting data, completing monthly reports and assisting with driving clients when needed. This position requires the ability to work independently, exercise sound judgment, time management, and initiative. Under the supervision of the SCM may conduct initial intake/screen applicants, complete the intake and application process, purchase and deliver items requested by the primary care manager, manage monthly care management contact logs, contact participant’s primary doctors for medical information, and document activities in the participant files. Desired qualifications include a Bachelor's degree in a human services discipline or two years of experience working with the elderly. SENIOR TRANSPORTATION PROGRAM (SONORA, CA) -PROGRAM COORDINATOR

The coordinator will oversee the day to day operations of senior non-emergency transportation for Tuolumne County. Duties will include client intake, the coordination of volunteer driver schedules, and maintaining of reporting for compliance. The ability to interface effectively with parishes, community, local seniors, and government agencies to provide effective communication and partnership building is required. The coordinator must have strong verbal and written communication skills. Prior experience working with senior populations is preferred. IMMIGRATION LEGAL SERVICES -CASE MANAGER

Case management duties include, but not limited to; interviewing clients regarding their legal immigration status, assisting clients with immigration forms and processes while also providing administrative duties. Candidate must be bilingual (English/Spanish) with strong interpersonal, written and oral communication skills as well as proficient in computer skills. Candidate will have a positive and professional attitude, flexible, attention to detail and sensitive to the needs of culturally and economically diverse client populations. While no immigration work experience is required, candidate will preferably completed courses/hold degree in Law, Criminal Justice, Sociology, Social Work or related field. This position is funded by a federal grant requiring the operation of a Naturalization program in Stockton and Modesto. Coordinator will work at both sites weekly totaling 40 hours. Stockton school is M &W 5:30 – 9:00 pm, Modesto School is T & TH 5:30-9:00 pm and all other hours worked at Stockton Office. Coordinator is responsible for the daily operation of Naturalization program. Duties include but not limited to; providing intake and revision of enrollment for each student, ensure each student file is updated with appropriate signed documents, test scores and level of class, preparing sign-in for attendance of Instructors/Teachers and students, will work closely with Program Manager in providing statistics/data of attendance, hours of class, CASAS testing and benchmark data of each student for reporting purposes. Coordinator will have strong verbal and written communication skills, including excellent interpersonal skills; will possess extremely strong project and time management skills, including demonstrated capacity to manage and prioritize competing projects, and complete assignments under deadlines; High level of organization, attention to detail, and follow-through; excellent judgment, dependable, punctual, and able to maintain a high level of confidentiality. Qualifications: Must be fluent in Spanish. Two years of office environment experience and/or social justice work; Intermediate proficiency with Microsoft Word, Publisher and Excel (includes facility with spreadsheets and tables), demonstrated experience and effectiveness in coordinating and organizing activities and program events. Under the direction of the Site Director, the SCM is responsible for planning, organizing, developing and implementing the principles and techniques employed in a comprehensive care management model for the frail elderly. The SCM position combines both administrative duties and clinical supervision responsibilities. Requirements include: A Master’s degree in social work, nursing, psychology, counseling, rehabilitation, gerontology, or sociology. Minimum of one year Supervisory experience. Minimum of two years’ experience working with the aging population (65 years and above). Minimum of one year care management experience. Knowledge of services available to seniors in the community. Strong interpersonal, organizational and communication skills. Must be able to occasionally work non-traditional hours, overtime, and travel as needed for trainings and conferences.
