ADMINISTRATION -FACILITIES COORDINATOR
Catholic Charities of the Diocese of Stockton is seeking to hire a Facilities Coordinator; this position is part-time (25 hr. /week). The Facilities Coordinator is responsible for managing the organizational relationships with the property managers of the Catholic Charities office facilities. The position is responsible for overseeing or providing necessary maintenance activities. The Coordinator assists in monitoring janitorial services of the offices and implements the injury, illness and prevention program. Candidates for this position must have three year’s work experience in facilities management or a related field. Computer skills are required, excellent communication skills, a valid driver’s license, and proven ability to work collaboratively with culturally diverse populations.

MULTIPURPOSE SENIOR SERVICES PROGRAM -NURSE CARE MANAGER

The Registered Nurse Care Manager will conduct in depth assessments/reassessments for participants and certify level of care. Will work closely with the MSSP Clinical Supervisor, Social Worker Care Managers, Physicians and other health care professionals. Participate in the development of care plans and implementing services; case recordings and reporting’s. Monitor participant on going needs and provide a support system. Qualifications included: A current California Registered Nurse’s License, Bachelor’s Degree in Nursing preferred. Experience working with seniors and with care management. Strong verbal/written communication skills and computer literate to effectively use computer software. SENIOR SERVICES PROGRAM -TRANSITIONAL CARE SPECIALIST
Catholic Charities of the Diocese of Stockton is seeking to fill a Part Time Transitional Care Specialist (TCS) position in Tracy, CA. The TCS will provide adults, seniors, and their caregivers with referrals to social service organizations to avoid re-hospitalization after a discharge from the hospital. The TCS will coordinate, advocate and provide services that will meet each client’s needs. This may require coordinating and assisting clients with scheduling transportation, medical appointments, medication delivery, in-home care, establishing follow-up care and/or obtaining other community resources. The TCS will attend meetings with funding sources and vendors, keep track of clients via data base, monitor readmissions, etc. The TCS must have strong verbal and written communication skills in English and Spanish, and knowledge of community resources. Healthcare experience highly desirable. This position requires a valid California Driver’s License and reliable transportation. Qualifications include: A Bachelor’s degree in Sociology, Psychology or social work or a minimum of two years’ experience in case management with adults and/or seniors. CALFRESH OUTREACH PROGRAM -APPLICATION ASSISTOR (STANISLAUS COUNTY)
Catholic Charities Nutrition Assistance Services has a part-time opening for a CalFresh Outreach-Application Assistor in the Modesto office. This position provides outreach activities to increase awareness of CalFresh benefits in Stanislaus County, in both English and Spanish. The position will require the incumbent to work 29 hours per week, and may include occasional evenings and weekends. NUTRITION ASSISTANCE SERVICES -SNAP-ED PROGRAM COORDINATOR (STOCKTON)
Catholic Charities Nutrition Assistance Services has a full-time opening for a SNAP-Ed Program Coordinator in the Stockton office. The ideal candidate will have knowledge of nutrition as it relates to low-income populations and food insecurity. The position will require the Coordinator to work 35 hours per week, and may include occasional evenings and weekends. The successful candidate will have demonstrated competence in facilitating workshops, excellent organization, strong leadership and interpersonal communication skills. Must be fluent in both Spanish and English, and be computer literate with proficiency in Microsoft Word, Excel, Publisher, and Power Point. VETFAM PROGRAM - CASE MANAGER
Catholic Charities of the Diocese of Stockton’s VETFAM Program is seeking a Full-time Case Manager for Tuolumne County to work out of our Sonora office. The Case Manager will work closely with the VETFAM Program Manager to locate affordable housing for homeless veterans currently living in Tuolumne County. Preferred qualifications include strong communication skills (both verbally and in writing) community outreach, HMIS data entry experience required, have a clean, DMV record with ability to travel to San Joaquin County, that we serve, at times.
